Parents must complete and/or bring the following:

• Complete Enrollment Agreement (can get a copy at the center or on website)

• The More You Know Form

• Up to date Immunization records

• Current physical for children 30 months and under

• Medication Consent Form

To be completed at the center:

• Tour the facility

• Review State licensing and Kids Cove LLC policies

• Pay registration and other applicable fees
